Kiosk | ![]() You may order a duplicate registration certificate with or without a decal. If the kiosk is not able to dispense the correct color of decal for the year requested, the duplicate registration/decal will be mailed. If you order a certificate only or the correct decal is available, the kiosk will print the document on the spot. The registration must be tied to your driver’s license record. You will be asked to fill-in the following information to verify you are the owner of the vehicle:

A duplicate Certificate of Registration or plate decal can be obtained for a fee of $6. Application for Duplicate Registration Certificate (VP 013) You must have a copy of your Nevada Evidence of Insurance Card. You may mail the application, card and a check, money order or a Payment Authorization (VP 205) to the address below. You may also fax the form, card and an authorization to (775) 684-4797. DMV Central Services If you need to change your address by mail, you must also complete a Change of Address Notification by Mail (DMV 22).
